Since 1982, Covenant House has supported more than 100,000 young people.<br></p><p><b>Overview:&nbsp;</b>This position is an exciting opportunity to be part of ground-breaking Shelter Diversion work. The Diversion Case Manager is a creative, out-of-the-box thinker, responsible for delivery and development of Covenant House Toronto’s Shelter Diversion program. The purpose of Shelter Diversion is to assist young people (ages 16-24) identify safe alternatives to staying in a shelter, so that experiences of homelessness are rare, brief and nonrecurring</p><p>The Diversion Case Manager will offer immediate, direct support to youth who are experiencing homelessness, or are at imminent risk of homelessness. Collaborating with young people, the Diversion Case Manager will explore alternatives to staying in shelter. This may involve engaging with young people’s family, friends and natural supports, and mediating with landlords and housing providers to help youth maintain or quickly access safe, stable housing. The Diversion Case Manager will offer short term, follow up Case Management to youth diverted from shelter to support the sustainability of their housing situation.</p><p>The role requires that services are delivered from a Client-Centered, Trauma Informed, Harm Reduction, Anti-Oppressive, Housing First for Youth, and Strengths/Resiliency based approach.</p><p><b>We are currently recruiting for 1 Full-time, Contract Diversion Case Manager. This is a replacement position within the organization.&nbsp;</b></p><p><b><u>Responsibilities:</u></b><br></p><ul><li>Engage with young people who are experiencing homelessness, or at imminent risk of homelessness, with a focus on problem solving their housing situation to identify and secure safe alternatives to a shelter stay</li><li>Engage family members, friends and chosen supports of youth to secure safe alternatives to a shelter stay</li><li>Liaise and mediate with landlords and housing providers to prevent immediate housing loss and/or support housing retention</li><li>Provide short term, follow up Case Management services to youth to support housing stability</li><li>Manage the distribution of emergency funding to young people to prevent housing loss, or secure safe housing options.</li><li>For young people who do not have Diversion options provide immediate navigation supports to access shelters, drop-in programs, and other resources</li><li>Facilitate group based activities, workshops and presentations as required</li><li>Work collaboratively within the Covenant House Toronto team, the youth shelter system and broader community</li><li>Work with stakeholders and partners to develop and promote this specialized youth service</li><li>Contribute to ongoing analysis, evaluation and quality improvement of the program</li><li>Maintain all required documentation including assessments, case notes, and evaluation tools and measures to support research and evaluation of Shelter Diversion.</li></ul><p><b><u>Qualifications</u></b></p><ul><li>Minimum BSW or equivalent in education and experience required. MSW preferred</li><li>Registered Social Worker in good standing with OCSWSSW preferred&nbsp; &nbsp;</li><li>3 years’ experience working with youth at-risk of and/or experiencing homelessness</li><li>Ability to advocate for, and promote homelessness prevention</li><li>Excellent ability to engage in collaborative problem solving conversations with young people, family members, natural supports, landlords and housing providers, with a demonstrated ability to generate effective solutions that meet youth’s needs&nbsp;</li><li>Previous experience making referrals and supporting young people access additional resources</li><li>Demonstrated ability in working with a trauma informed, harm reduction approach and from an Anti-Black racism, anti-oppression and culturally safe perspective/framework&nbsp;</li><li>Excellent communication (both oral and written) and organizational skills and ability to work independently, proactively, and collaboratively</li><li>Understanding of issues related to youth homelessness, 2SLGBTQ identities, mental health and substance use problems and disorders, and neurodiversity and disabilities&nbsp;</li><li>Knowledge of community resources and systems relevant to youth at-risk of, and experiencing homelessness</li></ul><p>Don’t meet every requirement? This role will support staff from an educational lens of benefits, entitlements and front-end user functionality.&nbsp;<br>This role works collaboratively with P&amp;C Employee Experience Specialist to ensure all areas such as training, policy communications, systems support and KPIs are effective and efficient. Since 1982, Covenant House has supported more than 100,000 young people.<br></p><p><b>Overview:</b><br></p><p>The Case Manager, Thrive Newcomer Program is responsible for developing and overseeing case management for youth assigned to their case management team and for providing case management leadership and mentoring to the team.</p><p>This position requires a high level of engagement with diverse populations, including youth who are newcomers, refugees, and those with precarious immigration status. A strong working knowledge of immigration systems, legal supports, settlement services, and barriers faced by newcomer youth is essential.</p><p>This role involves relationship building, assessment, collaborative planning, monitoring/evaluation, advocacy, and systems navigation support involving internal and external services and resources. The Case Manager will work collaboratively across our continuum of services, utilizing an integrated Client-Centered, Trauma-Informed, Harm Reduction Informed, AOP (Anti-Oppressive Practice), Housing First for Youth, Strengths/Resiliency-Based approach, with a focus on developmentally appropriate wellness and transition planning towards sustainable exits from homelessness.</p><p><b>We are currently recruiting for 2 Full-time, Permanent Thrive Case Managers. This is a new and replacement position within the organization.&nbsp;</b></p><p><b><u>Responsibilities:</u></b><br></p><ul><li>Provide ongoing case management engagement for youth assigned to the Case Manager and their team, with an emphasis on inclusive, culturally competent support for newcomer and immigrant youth.</li><li>Collaborate with youth to assess their needs, discuss options, and develop a case plan that reflects their strengths and lived experiences with sensitivity to cultural identity, migration history, and legal status.</li><li>Assess risks and develop safety plans in collaboration with youth (e.g., relational, mental health, substance use, immigration concerns),and coordinate proactive risk mitigation strategies with the Thrive team.</li><li>Provide mentorship and oversight to team members in implementing individualized case plans.</li><li>Maintain up-to-date, accurate, and professional case documentation that aligns with the standards and ethical guidelines set out by the Ontario College of Social Workers and Social Service Workers (OCSWSSW), including timely recording of assessments, case plans, progress notes, risk evaluations, service referrals, and case reviews.&nbsp;</li><li>Ensure all documentation reflects a client-centered, strengths-based approach, respects client confidentiality and informed consent, and upholds principles of transparency and accountability, and professional integrity.</li><li>Ensure case management includes appropriate referrals to settlement services, immigration legal supports, language access resources, and trauma-specific services for those impacted by forced migration or conflict.</li><li>Facilitate access to internal and external service providers, including supports related to physical/mental health, substance use, housing, and immigration pathways (e.g., legal aid, refugee/asylum claims, PR applications).</li><li>Coordinate case conferences and consultations, advocating for youth within systems such as IRCC, Legal Aid Ontario, and settlement agencies.</li><li>Maintain up-to-date knowledge of community resources, including newcomer and immigration-specific supports.</li><li>Participate actively in program development, interdepartmental collaboration, supervision, and continuous learning.</li><li>Contribute to quality improvement initiatives and evidence-based practice within the department.</li><li>Other additional duties as assigned&nbsp;</li></ul><p><b><u>Qualifications</u></b></p><ul><li>BSW or MSW degree, with 5 years of related experience, including at least 2 years working with youth experiencing homelessness.</li><li>Registration in good standing with the College of Social Workers and Social Service Workers.</li><li>Experience working with newcomer, refugee, and immigrant youth is strongly preferred.</li><li>Demonstrated understanding of immigration systems, legal protections, and barriers affecting youth with precarious status (e.g., human trafficking, unaccompanied minors, refugee claims, family reunification issues).</li><li>Strong commitment to an integrated, anti-oppressive, trauma-informed, harm reduction, and housing-first approach.</li><li>Proven ability to manage complex cases involving housing instability, substance use, mental health, and intersectional identities, including immigration-related vulnerabilities.</li><li>Excellent interpersonal, advocacy, and communication skills.</li><li>Familiarity with documentation practices, outcome measurement, and reflective supervision.</li></ul><p>Don’t meet every requirement?
